As previously communicated, effective immediately Texas has approved a new app/brochure for the RT85 and the 400 Series.
The new form number for the RT85 is RT85/GD SLTXP, and for the 400 Series the new form number is SWLB SLTXR P.
The cut-off date for new business written with the old app/brochure is Nov. 15, 2001. You can order updated supplies by contacting Agent Supply at (800)285-3676.

All applications for United American Insurance policies must be negotiated in person by the Writing Agent. Under no circumstances should applications be taken through the mail or over the phone.

Change In Underwriting
We have changed our Underwriting rule on underage health plans. The
GSP/HSXC series can now be issued with a major medical plan provided the deductible on the major medical plan is higher than the miscellaneous schedule on the
GSP/HSXC. For example, if the major medical (in force or pending) has a deductible of $10,000, the HSXC 7500 can be used (see other examples below).

Proof of the major medical deductible must accompany the application for
GSP/HSXC. If the major medical is applied for with another company at the same time as the
GSP/HSXC, a copy of the major medical application must be attached to the
GSP/HSXC. If the major medical is already in force, a copy of the policy schedule page must be attached to the
GSP/HSXC application.
